United Way Area Agency on Aging of Jefferson County: Request for Application

The United Way Area Agency on Aging of Jefferson County, a program of the United Way of Central Alabama, Inc., has been designated as the Area Agency on Aging for Jefferson County, effective October 1, 2016. It requests applications to provide 2017 Older Americans Act Title III B Social Service and Title III E Caregiver Services in Jefferson County beginning October 1, 2016.

The services requested are listed in this document.

Applicants may apply by submitting a Written Notice of Intent, in the form on Attachment B hereto, with required attachments, to the following email address: aaa@uwca.org

Letters of Intent are due by noon, (12:00 p.m.) on September 15, 2016. Questions may also be submitted to this email address.

After receipt of the Letter of Intent, applicants may receive more information and a Contract. The contract must be signed and emailed to the address above by 4:00 p.m., Monday, September 19, 2016. In addition, four hard copies of the contracts must be mailed to the following address and postmarked no later than September 19, 2016:

Date of contract period: October 1, 2016 through September 30, 2017, with up to four one-year renewal periods.

Submission of a Written Notice of Intent does not bind an organization to submit a Contract, and submission of a Contract does not guarantee selection for provision of services.

Written Notice of Intent must include the following:

  1. Proof of professional insurance/ bonding of employees/malpractice insurance
  2. Most recent audited financial statements
  3. Most recent federal income tax return or Alabama Certificate of Compliance
  4. List of services to be provided
