Senior Support Solutions
Case Management
Assessing needs, planning care, coordinating services, connecting seniors and families to resources, and offering encouragement and support.
Financial Assistance
Financial aid for rent, utility bills, mortgage payments, medication, transportation and for seniors who are the primary caretakers of children.
Health Education
Free exercise plans and programs for seniors and education on health and wellness topics shared through community center programs and newsletters.
Housing
Investigate complaints on behalf of residents at all long-term care facilities in Jefferson County and provide income-based housing for seniors.
Meal Delivery
Daily delivery of nutritious meals to homebound seniors in Jefferson County by dedicated community volunteers.
Medication Aid
Financial assistance for seniors and people with disabilities to help pay for necessary prescription medications.
Personal Care Services
Daily personal care and hygiene assistance, including light housekeeping and laundry, as well as meal prep and assistance with bathing and dressing.
Respite Care
Temporary substitutes for primary and family-member caregivers.
